Tian Ma Bian Chun Zhi Gan (TMBCZG) is a small molecule natural product extract derived from *Gastrodia elata* (Tianma), a traditional Chinese medicine. It is being developed by Peking Union Second Pharmaceutical Factory as a tablet formulation for the treatment of mild to moderate vascular dementia. Preclinical studies have demonstrated that TMBCZG can improve learning and memory abilities in animal models and reduce neuropathological damage. The drug has received approval for clinical trials from the Center for Drug Evaluation of China (CDE/CFDA) and is currently undergoing Phase II clinical evaluation. These trials assess cognitive outcomes using standardized measures such as the Vascular Dementia Assessment Scale-cognitive subscale (VADAS-Cog) and the Clinical Dementia Rating-Sum of Boxes (CDR-SB).
