Tianjing Tongluo Decoction (TJTLD) is a Traditional Chinese Medicine (TCM) formula developed by Dongzhimen Hospital of the Beijing University of Chinese Medicine, specifically indicated for the treatment of diabetic kidney disease (DKD). Based on the TCM theory of "collateral disease" (Luobing), the decoction aims to tonify the kidneys, nourish essence, and unblock the renal collaterals. It is a multi-herb combination typically including *Astragalus membranaceus*, *Rehmannia glutinosa*, and *Hirudo*. Pharmacological studies indicate that TJTLD exerts therapeutic effects by reducing proteinuria, improving glomerular filtration rates, and mitigating renal fibrosis. Its mechanism of action involves the inhibition of the TGF-β1/Smad signaling pathway, reduction of pro-inflammatory cytokines such as TNF-α and IL-6, and protection of podocyte integrity by regulating proteins like nephrin and podocin. It is primarily used as a hospital-made preparation in clinical settings in China.
