Tianlong Kechuanling (TL) is a Traditional Chinese Medicine (TCM) compound hospital preparation primarily utilized in clinical practice for the treatment of pulmonary heart disease (cor pulmonale) and its complications, such as chronic obstructive pulmonary disease-associated pulmonary hypertension (COPD-PH). Administered as a decoction, TL has been shown to improve pulmonary function and reduce right ventricular systolic pressure (RVSP) and hypertrophy. Its therapeutic mechanism involves the inhibition of endothelial cell pyroptosis by targeting the DNMT3A-GSDMD signaling pathway and the attenuation of pulmonary hypertension by suppressing endothelial-to-mesenchymal transition (End-MT) via the HIF-1α/VEGFA pathway. Additionally, it modulates inflammatory responses by reducing levels of pro-inflammatory cytokines including TNF-α, IL-1β, IL-6, and IL-18.
