Overview

Tiemonium methylsulphate is a quaternary ammonium antispasmodic agent traditionally used for the symptomatic treatment of visceral spasms in the gastrointestinal, biliary, and urinary tracts. It functions as a competitive antagonist at muscarinic acetylcholine receptors, thereby inhibiting parasympathetic-mediated smooth muscle contraction. More recently, research conducted by Amsterdam UMC Stichting has explored the repurposing of tiemonium methylsulphate for the treatment of atherosclerosis. In this context, the drug is hypothesized to modulate the inflammatory environment of the arterial wall by influencing macrophage polarization and reducing the formation of foam cells, which are critical drivers of plaque progression and instability. Because it is a quaternary ammonium compound, it has limited penetration of the blood-brain barrier, which minimizes central nervous system side effects.
