TIL-TCM is an autologous adoptive cell therapy consisting of tumor-infiltrating lymphocytes (TILs) that have been expanded and modulated ex vivo to favor a central memory T-cell (TCM) phenotype. Developed by Sizhen Wang, this therapy is specifically designed to treat advanced, relapse-refractory, or metastatic pancreatic cancer. The TCM phenotype is characterized by enhanced longevity, superior self-renewal capacity, and the ability to rapidly differentiate into potent effector cells upon re-encountering tumor antigens. These properties are intended to overcome the common limitation of poor T-cell persistence in the immunosuppressive tumor microenvironment of solid tumors like pancreatic cancer. The treatment protocol involves harvesting TILs from the patient's tumor tissue, expanding them using specific cytokines and culture conditions to enrich for the TCM population, and re-infusing them into the patient, typically following a lymphodepleting conditioning regimen of cyclophosphamide and nab-paclitaxel (Abraxane).
