Tumor-infiltrating lymphocyte (TIL) transfer combined with interleukin-2 (IL-2) is a form of adoptive cell therapy used primarily in the treatment of advanced solid tumors, most notably metastatic melanoma. The process involves surgically removing a tumor sample from the patient, isolating and expanding TILs ex vivo using high-dose recombinant human IL-2 (aldesleukin), and then infusing these activated cells back into the patient following a preparative lymphodepleting chemotherapy regimen. After infusion, additional high-dose IL-2 is administered to promote proliferation and activation of the transferred T cells within the patient. This combination leverages both cellular immunotherapy (via polyclonal tumor-reactive T cells) and cytokine support to enhance anti-tumor immune responses. Clinical studies have shown significant response rates in patients with advanced melanoma, including those previously treated with immune checkpoint inhibitors[1][3][4][5].
