Overview
Tilazem (diltiazem hydrochloride) is a non-dihydropyridine calcium channel blocker developed by Pfizer for the management of hypertension and angina pectoris. As a benzothiazepine derivative, it selectively inhibits the influx of calcium ions during membrane depolarization in cardiac and vascular smooth muscle. This inhibition occurs primarily at the L-type calcium channels (specifically the alpha-1 subunit), leading to peripheral and coronary vasodilation, reduced peripheral resistance, and decreased myocardial oxygen consumption. Additionally, Tilazem slows atrioventricular (AV) node conduction and prolongs AV node refractoriness, making it effective in controlling heart rate. It is typically administered orally in tablet form.
