Tilmicosin is a semi-synthetic macrolide antibiotic used exclusively in veterinary medicine. It is primarily indicated for the treatment and control of respiratory diseases in cattle, sheep, swine, and rabbits caused by pathogens such as Mannheimia haemolytica, Pasteurella multocida, Histophilus somni, Haemophilus parasuis, Mycoplasma hyopneumoniae, and Actinobacillus pleuropneumoniae[1][2][4][5]. Its mechanism of action involves binding to the 50S subunit of bacterial ribosomes at the peptidyl transferase center. This inhibits protein synthesis by blocking peptide bond formation and translocation during translation—resulting in bacteriostatic or bactericidal effects depending on concentration and organism[6][8]. Tilmicosin concentrates well in lung tissue and alveolar macrophages after administration. It is not approved for human use due to severe cardiotoxicity risks if accidentally injected into humans[2]. The drug was developed by Elanco.
