**TILT-123 + tumor infiltrating lymphocytes** is a combination immunotherapy in which the oncolytic adenovirus TILT-123 is used together with autologous tumor infiltrating lymphocytes (TILs) for cancer treatment. TILT-123 is a genetically engineered oncolytic adenovirus (Ad5/3-E2F-D24-hTNFα-IRES-hIL-2, igrelimogene litadenorepvec) encoding tumor necrosis factor alpha (TNFα) and interleukin-2 (IL-2), designed to enhance T-cell recruitment, activation, proliferation, and antitumor immune response. The capsid is modified (5/3 chimerism) for enhanced tumor tropism and avoidance of pre-existing immunity. When combined, TILT-123 improves tumor microenvironment immunogenicity, boosting the efficacy of adoptively transferred TILs by facilitating immune cell infiltration, activation, and tertiary lymphoid structure formation in refractory tumors. TILs are extracted from resected patient tumor tissue, expanded ex vivo, and reinfused between virus administrations.
