Timoptic XE is a once-daily, gel-forming ophthalmic formulation of timolol maleate, a non-selective beta1/beta2-adrenergic receptor–blocking agent, developed to lower intraocular pressure in patients with open-angle glaucoma or ocular hypertension.[1][2][17] The gellan gum–based vehicle forms a gel on contact with the tear film, prolonging ocular residence time and allowing effective intraocular pressure reduction over 24 hours with a single daily drop at 0.25% or 0.5% strength.[1][2][11] Timolol decreases aqueous humor production in the ciliary body, thereby reducing intraocular pressure, and as a topical beta-blocker can cause systemic beta-adrenergic blockade–related effects such as bradycardia, bronchospasm, and masking of hypoglycemia.[1][2][6] The product was originally developed and marketed by Merck and is now marketed in some regions by Elvium Life Sciences, with brand Timoptic XE discontinued in some markets while generic gel-forming timolol maleate products remain available.[1][2][4][11]
