Tiotropium bromide + theophylline is a combination therapy used primarily in the management of chronic obstructive pulmonary disease (COPD). Tiotropium bromide is a long-acting antimuscarinic bronchodilator that acts mainly on M3 muscarinic receptors in airway smooth muscle, leading to bronchodilation. Theophylline is a methylxanthine derivative and non-selective phosphodiesterase inhibitor that exerts its effects by relaxing bronchial smooth muscle and suppressing airway responsiveness. Clinical studies have shown that combining tiotropium with low-dose theophylline can significantly improve symptoms, general health status, and small airway function in patients with stable COPD compared to monotherapy[1][7]. This combination targets different pathways involved in bronchoconstriction, providing additive or synergistic benefits for patients who do not achieve adequate control with single-agent therapy.
