This therapeutic regimen consists of a sequential administration of intravenous tirofiban followed by oral dual antiplatelet therapy (DAPT), specifically aspirin and clopidogrel. Tirofiban is a small molecule, reversible glycoprotein IIb/IIIa receptor inhibitor that blocks the final common pathway of platelet aggregation. Aspirin acts as an irreversible inhibitor of cyclooxygenase-1 (COX-1), while clopidogrel is a thienopyridine that irreversibly inhibits the P2Y12 ADP receptor. This combination is being investigated by the Second Affiliated Hospital of Soochow University in the Phase 4 TiMIS (Tirofiban in Mild Ischemic Stroke) trial. The study evaluates whether initiating a 48-hour tirofiban infusion within 48 hours of symptom onset, followed by standard DAPT, improves functional outcomes (mRS 0-1) at 90 days in patients with mild noncardioembolic acute ischemic stroke (NIHSS score ≤5).
