Tisagenlecleucel is a CD19-directed, genetically modified autologous T cell immunotherapy, classified as a CAR-T cell therapy. It is made by collecting a patient's own T cells and engineering them to express a chimeric antigen receptor (CAR) that targets the CD19 antigen on B cells. Once infused back into the patient, these modified T cells recognize and kill cancerous B cells expressing CD19. The therapy includes costimulatory domains such as 4-1BB to enhance expansion and persistence of the CAR-T cells, and a CD3ζ signaling domain for T cell activation. Tisagenlecleucel is primarily indicated for relapsed or refractory B-cell precursor acute lymphoblastic leukemia (ALL) in patients up to 25 years old, relapsed or refractory large B-cell lymphoma in adults (including diffuse large B-cell lymphoma), and relapsed or refractory follicular lymphoma in adults after at least two prior therapies[3][5][7][8].
