Tislelizumab + IL-2 + capecitabine + oxaliplatin (also referred to as Tislelizumab + IL-2 + CAPOX) is an investigational combination regimen being evaluated as a neoadjuvant therapy for colorectal cancer. The regimen combines **tislelizumab** (Tevimbra), a humanized anti-PD-1 monoclonal antibody developed by BeiGene, with **interleukin-2** (IL-2), an immunostimulatory cytokine, and the standard **CAPOX** chemotherapy doublet consisting of **capecitabine** (an oral fluoropyrimidine prodrug) and **oxaliplatin** (a platinum-based alkylating agent). Tislelizumab works by blocking the PD-1 receptor on T cells, preventing its interaction with PD-L1 and PD-L2 to restore anti-tumor immune responses. IL-2 acts as a T-cell growth factor to further enhance the immune response. The chemotherapy components, capecitabine and oxaliplatin, induce DNA damage and inhibit tumor cell proliferation. This four-drug combination is currently being studied in a Phase 1/2 clinical trial for patients with left-sided colorectal cancer complicated by acute obstruction following loop colostomy.
