Titrated oral misoprostol solution is a liquid formulation of the synthetic prostaglandin E1 (PGE1) analog, misoprostol, being evaluated by China Medical University Hospital for labor augmentation. Misoprostol acts as an agonist at prostaglandin E receptors (primarily the EP3 subtype) in the uterine myometrium, which triggers intracellular calcium release and leads to potent uterine contractions. While traditionally administered as a tablet for gastric ulcer prevention or off-label for labor induction, this specific titrated oral solution allows for precise dose adjustments to achieve adequate uterine activity while minimizing the risk of uterine tachysystole. A Phase 4 randomized controlled trial (NCT02433106) compares this titrated oral regimen to the standard of care, intravenous oxytocin, in pregnant women experiencing inadequate uterine contractions during labor.
