TLN-499 is an orally bioavailable small molecule inhibitor of B-cell lymphoma-extra large (BCL-XL), developed by Treeline Biosciences. It is currently being evaluated in Phase 1 clinical trials, primarily in combination with the BCL-2 inhibitor venetoclax, for the treatment of relapsed or refractory solid tumors. BCL-XL is a member of the BCL-2 family of proteins that regulates apoptosis by inhibiting pro-apoptotic factors; its overexpression is a common mechanism of resistance to BCL-2 inhibitors and is prevalent in various solid tumors such as small cell lung cancer (SCLC) and neuroendocrine cancers. By selectively inhibiting BCL-XL, TLN-499 aims to sensitize tumor cells to apoptosis, particularly when used in conjunction with other apoptotic triggers like venetoclax, while potentially managing the dose-limiting thrombocytopenia historically associated with BCL-XL inhibition.
