TLP-02 is a small molecule drug candidate consisting of standalone cilastatin, a potent inhibitor of the enzyme dipeptidase-1 (DPEP-1), also known as dehydropeptidase-I (DHP-I). Developed by Arch Biopartners under an exclusive worldwide license from Telara Pharma, TLP-02 is specifically being investigated for the prevention and treatment of acute kidney injury (AKI) associated with sepsis. While cilastatin has historically been used in combination with the antibiotic imipenem to prevent its renal degradation, TLP-02 represents its development as a standalone nephroprotective agent. The mechanism of action involves blocking DPEP-1 on the renal proximal tubule cells, which prevents the recruitment of neutrophils and the subsequent inflammatory cascade that leads to tubular damage during septic shock. The program is currently in the preclinical stage of development.
