TLP-04 is a small molecule dehydropeptidase-I (DHP-I) inhibitor being developed by Telara Pharma. It is a repurposed formulation of cilastatin, an agent traditionally used to prevent the renal metabolism of the carbapenem antibiotic imipenem. Telara Pharma is developing TLP-04 for an undisclosed indication, leveraging cilastatin's potential nephroprotective and neuroprotective properties. The drug works by inhibiting the DHP-I enzyme (also known as Dipeptidase 1 or DPEP1) located in the brush border of the proximal tubule in the kidney and potentially in the central nervous system, thereby mitigating cellular damage and inflammation. As of early 2024, TLP-04 is in the preclinical stage of development.
