TM4SF20-ADC is an antibody-drug conjugate (ADC) designed to target transmembrane 4 L six family member 20 (TM4SF20), a tetraspanin-like protein that facilitates liver metastasis in pancreatic ductal adenocarcinoma (PDAC) by scaffolding the DDR1-IRS4 signaling complex. The conjugate consists of a TM4SF20-specific monoclonal antibody linked to deruxtecan (DXD), a potent topoisomerase I inhibitor. By binding to TM4SF20 on the surface of cancer cells, the ADC is internalized and releases its cytotoxic payload, leading to DNA damage and apoptosis. Preclinical studies presented at AACR 2026 demonstrate that TM4SF20-ADC effectively eliminates both primary PDAC tumors and established liver metastases in orthotopic mouse models, suggesting it as a potential therapeutic strategy for advanced pancreatic cancer.
