TMB-365-ADC is a first-in-class antibody-drug conjugate (ADC) being developed by TaiMed Biologics for the treatment of HIV-1 infection. The therapeutic candidate consists of TMB-365—a long-acting monoclonal antibody derived from the CD4-directed entry inhibitor ibalizumab—conjugated to an integrase strand transfer inhibitor (INSTI) payload. TMB-365-ADC utilizes a dual mechanism of action: the antibody component binds to the CD4 receptor on T cells to block viral entry, while the subsequent internalization of the ADC allows for the targeted intracellular release of the INSTI payload. This payload then inhibits the HIV-1 integrase enzyme, preventing the integration of the viral genome into the host cell's DNA. By combining entry inhibition with intracellular replication block, TMB-365-ADC aims to provide a highly potent and durable antiviral effect with reduced systemic toxicity.
