TmBU regimen

TmBU is an intensified multi-drug chemotherapy conditioning regimen developed by The First Affiliated Hospital of Soochow University. It is used as a preparative treatment prior to allogeneic hematopoietic stem cell transplantation (allo-HSCT) for patients with high-risk or relapsed/refractory acute leukemia, including acute myeloid leukemia (AML) and acute lymphoblastic leukemia (ALL). The regimen typically consists of thiotepa (T), mitoxantrone (m), and busulfan (BU). Thiotepa and busulfan function as potent DNA alkylating agents, while mitoxantrone acts as a DNA intercalator and topoisomerase II inhibitor. This combination is designed to enhance the anti-leukemic effect and reduce the cumulative incidence of relapse (CIR) compared to standard protocols like the modified busulfan/cyclophosphamide (mBUCY) regimen. It is currently being evaluated in a prospective, randomized controlled Phase 3 clinical trial (NCT06816134).
