Overview

TMG-123 is a novel small molecule glucokinase activator developed by Teijin Pharma for the treatment of type 2 diabetes mellitus. It selectively activates glucokinase (GK) in the liver and pancreas, enhancing glucose uptake and improving glucose tolerance without increasing plasma insulin levels. In preclinical studies, TMG-123 demonstrated greater efficacy than metformin and glibenclamide in animal models of both insulin-deficient and insulin-resistant diabetes, with sustained reductions in hemoglobin A1c (HbA1c) levels over chronic administration. Unlike earlier GK activators, it did not increase hepatic or plasma triglyceride levels during long-term use. However, toxicology studies in rats indicated that prolonged hypoglycemia induced by TMG-123 can cause irreversible impairment of spermatogenesis. Clinical development reached phase II before being discontinued[1][2][3][4].
