Tn-MUC1 CAR-T cells are an investigational adoptive cell therapy consisting of chimeric antigen receptor (CAR) T cells engineered to target Tn-MUC1, an aberrantly glycosylated (Thomsen-nouvelle/Tn glycoform) tumor-associated form of the membrane mucin MUC1. MUC1 is overexpressed and abnormally glycosylated in a wide range of malignancies, making Tn-MUC1 a highly tumor-specific antigen that is largely absent from normal tissues, thereby offering a favorable therapeutic window. The CAR construct typically comprises a Tn-MUC1-specific antigen-binding domain (scFv, often derived from the 5E5 antibody), a transmembrane domain, a costimulatory signaling domain (such as CD2 or CD28), and an intracellular signaling domain (such as CD3-zeta). This therapy has been evaluated in preclinical and early-phase clinical studies for various solid tumors and hematological malignancies, including pancreatic adenocarcinoma, triple-negative breast cancer, non-small cell lung cancer, ovarian cancer, and multiple myeloma.
