TNB-738 is a fully human biparatopic (bispecific) antibody designed to inhibit the enzymatic activity of cluster of differentiation 38 (CD38), an ecto-enzyme primarily expressed on immune cells. CD38 metabolizes nicotinamide adenine dinucleotide (NAD+) and related substrates, leading to reduced NAD+ levels associated with metabolic and inflammatory diseases, aging, and neurodegenerative disorders. By simultaneously binding two distinct epitopes on CD38 using two non-competing heavy chain-only antibodies in a bispecific format, TNB-738 potently inhibits CD38's enzymatic activity. This inhibition boosts intracellular NAD+ levels and sirtuin activities without depleting CD38-expressing cells due to its silenced IgG4 Fc region—differentiating it from other anti-CD38 antibodies like daratumumab or isatuximab. The drug is being developed for broad therapeutic potential in metabolic disorders, inflammatory diseases, autoimmune disorders, and immunological conditions associated with NAD+ deficiencies[1][2][4][5].
