TNFerade + capecitabine is a combination therapy consisting of TNFerade and capecitabine. - **TNFerade** is a gene therapy product that uses a replication-deficient adenovector to deliver the human tumor necrosis factor-alpha (TNF-α) gene directly into tumor cells. The expression of the TNF-α gene is controlled by a radiation-inducible promoter (Egr-1), allowing for localized production of TNF-α in response to radiotherapy. This results in potent local anti-tumor effects through direct cytotoxicity and immune activation, including CD8+ T cell-mediated responses[6][8][9]. - **Capecitabine** is an oral prodrug that is enzymatically converted to 5-fluorouracil (5-FU) in the body, where it inhibits thymidylate synthase and disrupts DNA synthesis, leading to cancer cell death. The combination has been investigated primarily for use in locally advanced rectal cancer as part of chemoradiotherapy regimens[1]. The rationale for combining these agents lies in their complementary mechanisms—capecitabine provides systemic cytotoxic chemotherapy while TNFerade enhances local immune-mediated and direct anti-tumor effects.
