Tocilizumab + aspirin is a combination of two drugs with distinct mechanisms of action, used together in clinical practice for certain inflammatory and cardiovascular conditions. Tocilizumab is a recombinant humanized monoclonal antibody that acts as an interleukin-6 (IL-6) receptor antagonist, inhibiting both soluble and membrane-bound IL-6 receptors to reduce inflammation. It is primarily indicated for autoimmune diseases such as rheumatoid arthritis, giant cell arteritis, systemic juvenile idiopathic arthritis, cytokine release syndrome (CRS), and has been approved for use in severe COVID-19 cases requiring supplemental oxygen or mechanical ventilation[1][4][5][7][8]. Aspirin (acetylsalicylic acid) is a small molecule nonsteroidal anti-inflammatory drug (NSAID) that irreversibly inhibits cyclooxygenase enzymes COX-1 and COX-2, leading to decreased synthesis of prostaglandins and thromboxanes; this results in anti-inflammatory, analgesic, antipyretic, and antiplatelet effects. The combination may be considered in patients with overlapping indications—such as those with autoimmune disease at risk for cardiovascular events—but should be monitored due to potential pharmacokinetic interactions where tocilizumab can increase the metabolism of aspirin[1]. No major direct interaction has been reported between these two agents[2].
