Overview

Tolamolol is a phenoxypropranolamine derivative and a cardioselective beta adrenergic receptor antagonist (beta-blocker). It preferentially inhibits myocardial beta adrenoreceptors, with potency equivalent to propranolol but with greater cardioselectivity. Tolamolol effectively lowers blood pressure in patients with mild essential hypertension and reduces plasma renin activity. It has been shown to control ventricular rate in supraventricular arrhythmias and reduce the frequency of ventricular ectopic beats. Tolamolol is particularly useful for patients who require cardiac beta blockade but have obstructive pulmonary disease, due to its selectivity. The drug does not exhibit significant direct cell membrane effects or intrinsic sympathomimetic activity[1][2][5]. Its primary mechanism of action is antagonism of beta adrenergic receptors, leading to decreased heart rate, reduced cardiac output, and lower blood pressure[1][2][5].
