Tolcapone is a potent, selective, and reversible inhibitor of the enzyme catechol-O-methyltransferase (COMT), classified as a nitrocatechol-type COMT inhibitor. It is used as adjunct therapy in the symptomatic management of idiopathic Parkinson's disease, typically in combination with levodopa and carbidopa. By inhibiting COMT both peripherally and centrally, tolcapone slows the breakdown of levodopa, increasing its bioavailability and prolonging its effect on dopaminergic stimulation in the brain. This results in improved control of motor symptoms associated with Parkinson’s disease. Tolcapone carries a risk of severe liver toxicity and requires regular monitoring during use[1][2][3][4][5].
