tolterodine + oxybutynin ER is a combination pharmacological treatment investigated for the management of refractory overactive bladder (OAB). This combination therapy utilizes two distinct antimuscarinic agents to achieve better control of bladder symptoms in patients who do not respond adequately to monotherapy. Tolterodine is a competitive muscarinic receptor antagonist that exhibits a degree of functional selectivity for the bladder over salivary glands, thereby reducing urinary urgency and frequency. Oxybutynin ER is an extended-release formulation of oxybutynin, which acts as a potent muscarinic antagonist and also exerts direct spasmolytic effects on the bladder detrusor muscle. The extended-release formulation of oxybutynin is designed to provide stable plasma concentrations and potentially reduce the incidence of side effects like xerostomia compared to immediate-release versions. This specific combination was evaluated in a Phase 4 clinical trial sponsored by Buddhist Tzu Chi General Hospital.
