Tongbi Huoluo Decoction (TBHLD) is a traditional Chinese medicine (TCM) herbal formula derived from Duhuo Jisheng Decoction. It is composed of thirteen herbs and is predominantly used in the treatment of orthopedic diseases, most notably knee osteoarthritis (KOA) and rheumatoid arthritis. TBHLD exhibits anti-inflammatory and analgesic effects, regulates cell proliferation and differentiation, and has antioxidant properties. Its mechanism involves inhibiting extracellular matrix (ECM) degradation in cartilage by regulating the expression of type I collagens (such as Col1a1 and Col1a2) and matrix metalloproteinases (Mmp2, Mmp13), leading to amelioration of cartilage degeneration in osteoarthritis. Core active compounds include salvianolic acid, folic acid, baicalin, and (E,E)-3,5-Di-O-caffeoylquinic acid, which act through modulation of NF-κB and MAPK pathways and other targets. The prescription is informed by both empirical clinical use in TCM and modern pharmacological analysis. TBHLD is administered orally in the form of a water decoction.
