Tongluo Huoxue Fang is a Traditional Chinese Medicine (TCM) herbal formula primarily developed for the treatment of ischemic stroke and related cerebrovascular disorders. The formula is designed to address the TCM syndrome of meridian obstruction and blood stasis by promoting blood circulation and unblocking collaterals. It typically comprises a standardized mixture of botanical extracts, including *Astragalus membranaceus*, *Salvia miltiorrhiza*, *Ligusticum chuanxiong*, and *Pheretima aspergillum*. Pharmacological studies indicate that the formula exerts neuroprotective effects through multiple mechanisms, including the inhibition of neuronal apoptosis via Caspase-3 regulation, reduction of neuroinflammation by lowering TNF-alpha and IL-6 levels, and improvement of cerebral microcirculation. It has been evaluated in several randomized controlled trials in China, often as an adjunctive therapy to standard antiplatelet or neuroprotective treatments in the acute and recovery phases of stroke.
