Overview

Topical infliximab is an ophthalmic formulation of the chimeric monoclonal antibody infliximab, currently under investigation by the Centre hospitalier de l'Université de Montréal (CHUM) for the treatment of sterile corneal melt. Sterile corneal melt is a severe complication of various autoimmune and inflammatory diseases, such as rheumatoid arthritis and Sjögren's syndrome, characterized by the progressive thinning and potential perforation of the cornea. Infliximab functions by binding with high affinity to both soluble and transmembrane forms of tumor necrosis factor-alpha (TNF-α), a potent pro-inflammatory cytokine that plays a central role in the inflammatory cascade leading to tissue degradation. By neutralizing TNF-α locally on the ocular surface, this 10 mg/mL topical solution aims to arrest the melting process and promote corneal stability while minimizing the systemic exposure and associated side effects typically seen with intravenous administration of the drug.
