Topical losartan is an ophthalmic formulation of the angiotensin II type 1 (AT1) receptor antagonist, currently being evaluated by the Universidad Autonoma de Nuevo Leon for the treatment of corneal scarring fibrosis. While losartan is traditionally used as an oral antihypertensive, research has demonstrated that the renin-angiotensin system (RAS) plays a significant role in ocular wound healing. Specifically, losartan has been shown to inhibit the transforming growth factor-beta (TGF-β) signaling pathway, which is the primary driver of myofibroblast differentiation and subsequent fibrotic scarring in the cornea following injury or surgery. By blocking AT1 receptors locally, topical losartan aims to prevent or reverse corneal opacification and improve visual outcomes in patients with stable corneal scars.
