Topical methotrexate is an immunomodulatory formulation of the folate antimetabolite methotrexate, currently being investigated by the Jinnah Postgraduate Medical Centre for the treatment of localized alopecia areata. As a dihydrofolate reductase inhibitor, methotrexate interferes with DNA synthesis and repair, but in the context of autoimmune skin conditions like alopecia areata, it is utilized for its ability to suppress immune-mediated inflammation directed against hair follicles. This specific 1% gel formulation is being evaluated in a Phase 4 clinical trial to compare its efficacy and safety against the standard treatment, minoxidil 5% spray, in patients with well-defined patches of non-scarring hair loss.
