Topiramate + empagliflozin is a combination of two small molecule drugs being investigated for the treatment of obesity. Topiramate is an anticonvulsant approved for epilepsy and migraine prevention, known to reduce body weight by decreasing calorie intake and altering taste perception. Its mechanisms include stimulation of GABA-A receptor activity, inhibition of voltage-dependent sodium channels, reduction in glutamate activity at AMPA and kainate receptors, and inhibition of carbonic anhydrase isozymes[6]. Empagliflozin is a sodium-glucose co-transporter 2 (SGLT2) inhibitor approved for type 2 diabetes mellitus; it lowers blood glucose by promoting urinary glucose excretion[3]. Clinical studies have shown that the combination leads to greater weight loss than either drug alone or placebo in overweight or obese individuals without diabetes[1][2][5][8]. The combination also improves lipid profiles and reduces visceral obesity. This therapy remains investigational for obesity.
