Overview
Topiramate + insulin refers to the therapeutic combination of the anticonvulsant topiramate and the metabolic hormone insulin. Topiramate is a sulfamate-substituted monosaccharide that exerts its effects through the blockade of voltage-dependent sodium channels, enhancement of GABA-A receptor-mediated inhibitory neurotransmission, and antagonism of AMPA/kainate glutamate receptors, alongside weak inhibition of carbonic anhydrase. Insulin is a peptide hormone that regulates glucose metabolism by binding to the insulin receptor, facilitating glucose uptake in muscle and adipose tissue while suppressing hepatic glucose output. This combination has been primarily investigated in clinical research to mitigate the weight gain frequently associated with intensive insulin therapy in patients with type 2 diabetes, leveraging topiramate's known weight-loss properties to counteract the anabolic effects of insulin.
