Topiramate and phenytoin are both antiepileptic (anticonvulsant) drugs used primarily for the management of seizure disorders. Topiramate is also indicated for migraine prophylaxis and, in combination with other agents, for chronic weight management. Topiramate acts by modulating voltage-dependent sodium channels, enhancing GABA-A receptor activity at non-benzodiazepine sites, inhibiting AMPA/kainate glutamate receptors, and weakly inhibiting carbonic anhydrase isozymes. Phenytoin primarily stabilizes neuronal membranes by promoting sodium efflux from neurons, thereby reducing excitability. When used together in patients with epilepsy, these drugs may have pharmacokinetic interactions but generally do not cause clinically significant changes at recommended doses[5]. Both drugs are small molecule anticonvulsants developed to control seizures; topiramate was discovered during research into antidiabetic agents but repurposed after its anticonvulsant properties were found[8].
