Topotecan + cisplatin is a combination chemotherapy regimen used primarily for the treatment of advanced, recurrent, or persistent cervical cancer. Topotecan is a semi-synthetic derivative of camptothecin and acts as an inhibitor of Topoisomerase I, an enzyme essential for DNA replication. By stabilizing the Topoisomerase I-DNA complex and preventing religation of single-strand breaks, topotecan induces lethal double-stranded DNA breaks during replication, leading to apoptosis in rapidly dividing cancer cells[6][8]. Cisplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks and inhibits DNA synthesis and function. The combination has demonstrated improved survival outcomes compared to cisplatin alone in patients with advanced cervical cancer[5]. This regimen is indicated for stage IVB, recurrent or persistent carcinoma of the cervix not amenable to surgery or radiation therapy[3][5].
