Overview

The combination of topotecan and vinorelbine is a non-platinum containing chemotherapy regimen investigated for various cancers, particularly lung cancer. Topotecan is a topoisomerase I inhibitor that binds to the topoisomerase I-DNA complex, preventing DNA religation, leading to double-stranded DNA breaks and apoptosis. It acts as an uncompetitive inhibitor by binding to the enzyme-substrate complex and intercalating between DNA base pairs. Vinorelbine is a semi-synthetic vinca alkaloid that targets microtubules. It inhibits microtubule assembly by binding to β-tubulin subunits, reducing microtubule dynamics, preventing proper mitotic spindle assembly, and blocking cell division. This combination offers a potentially synergistic approach due to their distinct mechanisms (topotecan affecting DNA replication and vinorelbine disrupting microtubule function). It has been studied in clinical trials, including a phase I dose escalation study for relapsed or refractory non-small cell lung cancer (NSCLC) or small cell lung cancer (SCLC), demonstrating manageable toxicities and some clinical activity. It has also been integrated into more complex regimens like TVT and TVTC for acute leukemia.
