Overview

Topsalysin is a targeted prodrug consisting of a recombinant modified form of the Aeromonas protoxin proaerolysin, engineered to be selectively activated by prostate-specific antigen (PSA), an enzyme produced in high levels by prostate cancer and hyperplastic prostate cells. Upon activation by PSA, topsalysin forms stable transmembrane pores in the cell membrane, leading to localized cell death and tissue disruption specifically within the prostate. This mechanism allows for precise ablation of prostate tissue while sparing neighboring tissues and nerves. Topsalysin is delivered locally via intraprostatic injection under ultrasound guidance. It has been investigated primarily for benign prostatic hyperplasia (BPH) and localized low- to intermediate-risk prostate cancer[1][2][4][6].
