Indapamide is noted for its efficacy in reducing systolic blood pressure with fewer metabolic side effects compared to traditional thiazides[1][9]. - **Hydrochlorothiazide**: A classic thiazide diuretic that inhibits sodium reabsorption at the distal tubules, resulting in increased excretion of sodium and water as well as potassium and hydrogen ions. It is widely used for hypertension but may be less potent than indapamide or chlorthalidone[3][9]. - **Doxazosin**: An alpha-1 adrenergic receptor antagonist (alpha-blocker) that causes vasodilation by relaxing vascular smooth muscle, thereby reducing peripheral resistance and lowering blood pressure. The combination leverages multiple mechanisms—diuresis via different nephron sites (loop plus distal convoluted tubule) along with direct vasodilation—to achieve additive antihypertensive effects. Such multi-drug regimens are sometimes considered for resistant hypertension when monotherapy or dual therapy fails to achieve target blood pressures[2][8].
