This combination regimen consists of the PD-1 inhibitor toripalimab and a sequential chemotherapy backbone of epirubicin, cyclophosphamide, nab-paclitaxel, and carboplatin. It is being investigated for the neoadjuvant treatment of triple-negative breast cancer (TNBC), specifically following high-intensity focused ultrasound (HIFU) induction in a study sponsored by The First Affiliated Hospital with Nanjing Medical University. Toripalimab is a humanized IgG4 monoclonal antibody that blocks the interaction between the PD-1 receptor on T cells and its ligands, thereby enhancing the immune system's ability to detect and destroy cancer cells. The chemotherapy components—epirubicin (an anthracycline), cyclophosphamide (an alkylating agent), nab-paclitaxel (an albumin-bound taxane), and carboplatin (a platinum-based agent)—work through various cytotoxic mechanisms including DNA intercalation, DNA cross-linking, and microtubule stabilization to inhibit tumor cell proliferation and potentially increase the release of tumor antigens, synergizing with the immunotherapy.
