Toripalimab is a humanized IgG4 monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor. By blocking the interaction between PD-1 and its ligands, PD-L1 and PD-L2, it restores the ability of T-cells to recognize and attack tumor cells. In the specific clinical context of genitourinary small cell carcinoma (GUSCC), toripalimab is being investigated as a first-line treatment in combination with etoposide and platinum-based chemotherapy (either cisplatin or carboplatin), a regimen often referred to as EP/EC. This combination therapy aims to leverage the cytotoxic effects of chemotherapy alongside immune checkpoint inhibition to treat locally advanced or metastatic forms of this rare and aggressive malignancy. The development of this specific regimen is currently being led by Sun Yat-sen University Cancer Center through a Phase II multicenter trial.
