Overview
Toripalimab is a humanized IgG4 monoclonal antibody that targets the PD-1 (programmed death-1) receptor, functioning as an immune checkpoint inhibitor to restore anti-tumor T-cell activity. In the specific clinical context led by Peking University Cancer Hospital, toripalimab is being evaluated in combination with the FOLFSIM regimen for the treatment of advanced or metastatic neuroendocrine carcinoma (NEC). The FOLFSIM regimen consists of Simmtecan (a formulation of irinotecan), 5-fluorouracil (5-FU), and leucovorin (l-LV). This combination therapy aims to leverage the cytotoxic effects of chemotherapy—specifically topoisomerase I inhibition by irinotecan and thymidylate synthase inhibition by 5-FU—alongside the immune-potentiating effects of PD-1 blockade to improve outcomes in patients with high-grade neuroendocrine malignancies.
