This neoadjuvant immuno-chemotherapy regimen consists of the anti-PD-1 monoclonal antibody toripalimab combined with a sequential chemotherapy backbone of paclitaxel followed by epirubicin and cyclophosphamide. It is currently being evaluated in the Phase 2 NEOTRIO-HR clinical trial, sponsored by the Second Affiliated Hospital, School of Medicine, Zhejiang University. The regimen is specifically targeted at patients with high-risk, lymph node-positive, hormone receptor-positive (HR+), and HER2-negative breast cancer. Toripalimab works by blocking the PD-1/PD-L1 pathway to restore T-cell mediated anti-tumor immunity, while the chemotherapy components provide direct cytotoxic effects through microtubule stabilization (paclitaxel), DNA intercalation and topoisomerase II inhibition (epirubicin), and DNA alkylation (cyclophosphamide). The trial is also investigating the potential synergy of adding intensified radiotherapy (tumor boost) to this combination.
