Total Therapy II

Total Therapy II (TT2) is an intensive, multi-phase treatment protocol for newly diagnosed multiple myeloma, developed by the Myeloma Institute for Research and Therapy at the University of Arkansas for Medical Sciences (UAMS). The regimen builds upon the earlier Total Therapy I by incorporating the anti-angiogenic agent thalidomide and utilizing tandem autologous stem cell transplantation (ASCT). The protocol typically includes induction with VAD (vincristine, doxorubicin, and dexamethasone), stem cell mobilization and collection, tandem high-dose melphalan with ASCT, consolidation with DCEP (dexamethasone, cyclophosphamide, etoposide, and cisplatin), and maintenance therapy. In clinical trials such as UARK 98-026, supportive care measures like Epoetin alfa and exercise programs were also evaluated within the framework of this intensive chemotherapy and transplant regimen to manage treatment-related anemia and improve patient outcomes.
