Overview
TQB2868 + anlotinib + albumin-bound paclitaxel + gemcitabine is an investigational combination regimen being developed for the first-line treatment of metastatic pancreatic ductal adenocarcinoma (mPDAC). The regimen combines TQB2868, a bifunctional fusion protein targeting programmed cell death protein 1 (PD-1) and transforming growth factor-beta (TGF-β), with anlotinib (catequentinib), an oral multi-targeted receptor tyrosine kinase inhibitor, and a standard chemotherapy backbone of albumin-bound paclitaxel (nab-paclitaxel) and gemcitabine. This combination leverages a triple synergistic mechanism of action—combining dual-pathway immunotherapy, anti-angiogenesis/targeted therapy, and cytotoxic chemotherapy—to overcome the highly immunosuppressive and dense desmoplastic stroma characteristic of pancreatic tumors. Clinical trials, including Phase II and Phase III studies, have demonstrated promising antitumor activity, with significant improvements in objective response rate (ORR) and progression-free survival (PFS) compared to chemotherapy alone.
