**TRAIL-modified adipose-derived stem cells** are human adipose-derived mesenchymal stem cells that have been genetically engineered to express tumor necrosis factor-related apoptosis-inducing ligand (TRAIL), a member of the TNF superfamily. The purpose of this modification is to enable these stem cells to deliver TRAIL directly to tumor environments, exploiting both the homing capacity of adipose-derived stem cells (ADSCs) for tumors and the selective apoptotic activity of TRAIL on cancer cells while sparing normal cells. The secreted TRAIL protein from transfected ADSCs induces caspase-mediated apoptosis in malignant cells (notably melanoma and various solid tumor lines) via activation of death receptors, downregulation of survival pathways such as PI3K-AKT, and activation of caspase-3, -4, -8, and -9[1]. This approach is an experimental targeted cell-based gene therapy, not approved or marketed, and remains at the preclinical research stage.
