Overview

Tramadol + diclofenac is a fixed-dose combination analgesic used for the management of moderate to severe pain, including acute musculoskeletal pain, postoperative pain, and acute flares of osteoarthritis or rheumatoid arthritis[1][2][8]. This combination leverages two complementary mechanisms of action for synergistic pain relief. Tramadol is an atypical opioid that acts as a μ-opioid receptor agonist and inhibits the reuptake of serotonin and norepinephrine in the central nervous system, reducing afferent pain signaling and amplifying inhibitory pathways[1]. Diclofenac is a non-steroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX)-1 and COX‑2 enzymes, thereby blocking prostaglandin synthesis responsible for inflammation and nociceptive pain[6]. The combination provides both immediate (from tramadol) and sustained (from diclofenac) analgesia. Clinical studies have shown this fixed-dose combination to be effective with a favorable safety profile compared to other combinations such as tramadol-paracetamol[1][2].
