Overview

Trandolaprilat is the active metabolite of trandolapril, an angiotensin-converting enzyme (ACE) inhibitor used primarily in the management of hypertension, congestive heart failure, and to improve survival following myocardial infarction. Trandolapril itself is a prodrug that is converted in the liver to its biologically active form, trandolaprilat. The mechanism of action involves inhibition of ACE, which prevents the conversion of angiotensin I to angiotensin II—a potent vasoconstrictor—thereby reducing blood pressure by relaxing blood vessels and decreasing aldosterone secretion. This leads to diuresis and natriuresis with a mild increase in serum potassium[5][6]. Trandolapril/Trandolaprilat may also slow progression of renal disease in hypertensive patients with diabetes mellitus. Developed by Knoll and manufactured by Abbott.
